Certified Teacher (Math and Reading) (Part-Time)

Under the supervision of the project director, provide and assist in the delivery of excellent education to middle school-aged students in the Brookland-Lakeview 21st Century COLA Program (Afterschool Program and Summer Camp). The programs offer homework assistance, tutoring, academic enrichment with emphasis on Math and ELA, character and physical education, and classes/workshops for parents.

Community Health Worker

The Community Health Worker (CHW) will recruit and assist with the implementation of the NDPP. The NDPP is designed to prevent type 2 diabetes in people diagnosed with prediabetes or at risk for type 2 diabetes by emphasizing the need to make lasting lifestyle changes

Bilingual Community Health Worker (Part-Time)

Engage African American and Hispanic families with diabetes living in Cayce, West Columbia, and surrounding Richland and Lexington County communities to participate in the Diabetes Intervention Program (DIP) and implement the Intensive Lifestyle Intervention and booster sessions, facilitate the Lifestyle Incentive Program, and maintain engagement with participants via social media and text messaging.

Case Manager

Provides crisis intervention and case management services for clients. Plans, develops and assists with the day-to-day operations of the social services office. Guides and assists clients in developing measurable goals and objectives to increase financial and housing stability. Identifies and advocates with community resources to assist clients in accessing necessary and helpful resources. Ensures compliance with grants and other policies and procedures.

Activity Coordinator

 The Activity Coordinator is responsible for the creation and implementation of the activity schedule, both instructional and recreational. The Activity Coordinator will be a part of the Brookland-Lakeview 21st Century COLA Program instructional team, along with the teachers and the 21st Century COLA Program staff. The Activity Coordinator will report to the Project Director.

Instructional Assistant Position – Brookland-Lakeview COLA Program

Works with teachers and parents to address academic deficiencies and behavior challenges of students at Northside Middle School. The assistant will become part of the Brookland-Lakeview COLA Program instructional team. The instructional team will be comprised of the classroom teacher and school staff, parents, the 21st Century Program staff, the Program Advisory Committee, and the student. The Program’s principal goal is to help each student realize his/her potential.
